Medication Name (Generic and Brand): Ceftriaxone (Rocephin)	Order (including the dose, route and frequency) 1-2 g IV/IM once daily or divided q12h ``` Max: 4 g/day	Mechanism of Action: Third-generation cephalosporin antibiotic. Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, causing cell wall death.
Indication (Why is it given?) Treatment of serious bacterial infections including: - Pneumonia - Meningitis - Sepsis - UTIs - Skin and soft tissue infections - Intra-abdominal infections - Gonorrhea	Side Effects: Common: Diarrhea, injection site reactions, rash, elevated liver enzymes Serious: C. difficile colitis, hemolytic anemia, hypersensitivity reactions, anaphylaxis, biliary sludge/pseudolithiasis Drug-Drug Interactions: Calcium-containing IV solutions (precipitates in neonates) Warfarin (increased INR) Probenecid (increased ceftriaxone levels)	Nursing Considerations (Assessment) Monitor for allergic reactions Assess renal and hepatic function Check CBC, culture and sensitivity Monitor for signs of superinfection Assess injection site for phlebitis Teaching: Report signs of allergic reaction Complete full course of therapy Report severe diarrhea May cause dizziness Evaluation (Effectiveness): Resolution of infection signs/symptoms Negative culture results Normal vital signs and WBC count
Drug-Food Interactions: No significant food interactions. Can be given without regard to meals when given orally (IM/IV routes most common).
