Mr. Thomas Jameson - Day 1

Age: 69  |  PMH: Atrial Fibrillation, Type 2 Diabetes, HTN

Reason for Admission: Right-leg swelling and pain for 2 days. Ultrasound confirmed DVT. Negative initial PE on CT.

Thomas is alert and oriented but anxious. Mild SOB reported, no chest pain. Calf pain 6/10.

Plan: IV Heparin drip (weight-based, aPTT goal 60–80), bridging to oral Warfarin once therapeutic.
