Step-by-Step NCLEX Reasoning

This interactive tool will guide you through an assessment. For each category, review the definitions provided and select the score that best fits the client’s status.

1. Evaluate Sensory Perception

Select the option that best describes the client’s sensory status:


View Selection Criteria
  • Very Limited (2): Responds only to painful stimuli or has a sensory deficit involving half or more of the body; communication about pain is significantly restricted.
  • Slightly Limited (3): Responds to verbal commands but may not always be able to communicate discomfort or need to turn; has some sensory impairment in 1 or 2 extremities.

2. Evaluate Moisture

Select the option that best describes the client’s skin moisture:

  • Constantly Moist (1): Skin is kept moist almost constantly by perspiration, urine, etc. Linen must be changed multiple times per shift.
  • Very Moist (2): Skin is often, but not always moist. Linen must be changed at least once per shift.

3. Evaluate Mobility

Select the option that best describes the client’s mobility:

  • Very Limited (2): Makes occasional/slight changes in body or extremity position but cannot make frequent or significant changes independently.
  • Slightly Limited (3): Makes frequent, though minimal, changes in position without assistance.
